Appointments

For an appointment, please call (972) 547-0352. You may also request an appointment using our convenient on-line Appointment Request form. Once we recieve your appointment request, we will call you to confirm your appointment.

Office Hours

Monday - Friday, 8am - 5pm, lunch daily from 12pm -1:15pm..

In many cases, appointments are available the same day or next day.

To your first visit, please bring a picture I.D. and your insurance card. Arrive 30 minutes early to complete new patient paperwork. If you would prefer, the new patient paperwork can be downloaded and filled out prior to your visit. Please arrive 15 minutes early if you have the paperwork completed prior to your appointment.

In consideration of other patients and our staff, if you are going to be late for your appointment, please call our office so that we may accommodate other patients or re-schedule your appointment.

Cancellations

If you cannot make your scheduled appointment, we require 24 hour advance notice. No-show appointments may be charged a $25 fee for the missed appointment.
